Please ensure that your healthcare provider either gives to you or shares with us any pap smear and mammogram results, as well as the results of any recent bloodwork.
To offer you the best care we can, it’s important for us to know about any medications or supplements you’re taking. Please bring them with you to your first appointment, or bring a list of medications from a pharmacy.
